Travel Vaccinations

If you are travelling to a destination where vaccinations may be advised, you must complete a Travel Vaccination Assessment form for each person travelling.

Then you will need to contact the surgery reception to make an appointment for each person travelling. This appointment is to look at the vaccinations recommended for your destination, and plan your vaccination schedule.

Please bring your completed form with you.

The surgery can administer Tetanus/Diphtheria/Polio, Hepatitis A and Typhoid vaccines (for which there is no charge) and issue a private prescription for Malaria tablets.

If other vaccinations are recommended, you will be advised to attend a specialist Travel Vaccination Clinic for these.

This process should be started at least 8 weeks before travel.
