All requests should include your name, date of birth, contact details and medication required.
Holidays / Early Prescriptions - If you are going on holiday and require additional medication or an early prescription:
- For Paper Requests - please write the revised collection date and medication required
- For On-line Requests - please use the 'notes field' to indicate the revised collection date and medication required
NB Prescriptions are issued on a 28-day basis. Any requests received more than 7 days before they are due may not be processed.
Upon receipt of the request, a repeat prescription will then be issued. For any items not on repeat or needing a review, a request will be sent to your GP who will take the appropriate action. When signed, your prescription will then be made ready for you, or your pharmacy, to collect from us at reception, or it can be sent electronically straight to your nominated pharmacy.
We do not have the facility to automatically re-order your repeat prescription each month, it is your responsibility to ensure you do so. Please allow 48 hours (2 working days) for processing before asking to collect it. If you have attended a hospital consultation, or have had a hospital admission, we require written confirmation of any changes to your medication before we can update your repeat medication list. Again, please allow at least 48 hours (2 working days) for any changes to be processed before the prescription can be collected. Please note it is your responsibility to inform your pharmacy if your repeat medication list changes in any way.
All medication is subject to review by your doctor and you will be asked periodically to make an appointment with him/her before you can be given another prescription.
